Dr. Dunbar participates with most of the major insurance companies. Please be sure to bring your insurance card with you to your appointment, and if appropriate, a primary care physician referral.

Payment Plans

Should you need a payment plan, please discuss this with us at the time of your first visit.

Job-Related Injuries

If your condition is job-related, please let us know during your initial call, and be prepared to supply us with the following specific information so that we may obtain prior approval for treatment and for payment of fees:

• Employer's name, address and phone
• Date of injury
• Worker's compensation insurance company
• Claim number if known
